Is this something that Apple could prevent, or would have to sign off on? Is only the smart watch tech itself be patented, or also design elements like the straps? Thanks!

You would have to see precisely what patents APPLE has in connection with their watch - they may protect the watch per se and not the strap, they may have design patents for the watch and strap - consult a patent professional to do a freedom to operate (FTO) search.
