If a provisional patent is filed in the USA, but the full application is never filed, does the provisional patent become public knowledge?

Not unless you refer to it in another USPTO document. It also becomes public when the referring document becomes public. In this fashion, most provisionals become public when the non-provisional using it as the base for a prior-art date become public. You can see them in the 'Continuity Data' in PAIR.
