If I sign a "Disclosure of Inventions" agreement, does this mean that my employer owns an app I create on my own time?

I like to code in my free time and create my own software which based on my own ideas, not on my work for my employer. I also create artwork and design for it. If I signe the agreement does that mean that my employer can come after me for my own work ?
